This should have all been explained with what you got from
the JR rail people but: Go to the JR travel center at Narita
or Kansai and exchange the voucher for your pass and ticket.

(Disclaimer: I've never actually used a JR Rail Pass, but that is how 
I understand it works and is how it works in the rest of the world.)
