Here's another revision, this time with a much smaller diff. It changes
around the meaning of the 'host' as per Graham's advice to mean what it did
in RFC 1738 and 1630.

I've kept the 'host' syntax rule even though it's explicitly a FQDN (better
served by the 'reg-name' rule), because it's always been 'host'. Hopefully
that's not controversial.

The other TODOs still remain, and I'll try to work on them over the weekend
or early next week.
