This update addresses comments from Dale Worley, as summarized in the
email just sent to the list.

Chairs: This requires another WGLC. While the changes were clarifications,
they include changes in normative language, namely the mapping of
S/E/I/D/B bits from various codec payloads. H265, H264-SVC, H264 (AVC),
and VP8 experts should review these changes for correctness.
