Dear All,

The main changes (in addition to some other, basically editorial, changes) compared to -00 are:
- Added the media type parameter dec-parallel-cap that was agreed in Berlin
- Added media type parameters max-fps, max-tr, and max-tc as agreed on the reflector
- Added an informative note on detection of the last NAL unit of an AU after the semantics of the M bit as agreed on the reflector
