I have updated the draft based on Robert's feedback and my responses sent to
the list previously.   There are lots of edits, so Diff2 gives the best
view:

A few additional notes:

- I added some text about dialog matching which we need to be sure is
correct - see 5.2.1.

- There was a question about the ABNF changes and the SALUD work.  Since the
SALUD work is just on the Alert-Info URN, there are no ABNF changes planned
to RFC 3261.

- I added text relating to appearance handling during call transfers.  See
5.3.1, 5.3.2, and 5.3.3.
