Please file a bug at http://bugreporter.apple.com and include the full 
protocol traces you have (if possible complete ones that show the initial 
request line etc). Also, please use the Console.app to check for any 
messages coming from iCal as you try to setup the account. Attach protocol 
traces and console logs to the bug report, then email me the resulting bug 
number privately.

At first glance what you are doing should work but there are some things 
that are certainly different from what other servers typically do.

Also, if you have a way to make a test server available with test accounts 
that could be used, that may help in debugging this.
