Dear Mark: With limited knowledge of language, it seems to me that Fragmentation, Consolidation, Centralization, etc. and eventually Monopoly are manifestations from a series of behaviors with varying degrees of intensities due to the natural trend that a "free-market" environment tends to foster. I believe that, beyond clarifying the terminology and taxonomy, we need to be conscious about the above fundamental human nature in analyzing the issues and seeking the solutions by focusing on preserving and instituting certain mechanisms that can counter the trend.
