This update should address all issues raised during last call and issues
raised during secdir review and genart review. It also updates the
examples and references (note: it might be useful for DNS-COOKIES,
TCP-KEEPALIVE and QUERY-CHAIN to proceed as bundle so they can reference
each other by RFC number)
