I've seen working groups fail to adequately address comments and
then have it come back to bite them in later review or even when
the IESG is reviewing a document.  I've also seen them completely
ignore comments, but haven't seen one ignore comments from
someone with a formal role.

The thing is that the reasons for them doing so vary, from the
comments being muddled and irrelevant to the working group being
insular and overly committed to the wrong path.  I would tend to
look for a chairing failure when it happens.
