This draft covers the issues raised during the working group meeting. 
They are as follows:

  * Issue 20: s/IAOC/IASA/.  This is done.
  * Adjustment to back matter relating to late changes.
  * Changes to travel barriers, based on working group discussionn

In addition, the chairs have directed that a contributors section be
created, and authors moved to that section.  Dave Crocker has requested
that he not be listed in that section, but in Acknowledgments instead.
