This revision replaces citations of the work-in-progress
drafts for rfc5666bis and rpcrdma-bidirection with RFCs
8166 and 8167, and replaces the citation of the InfiniBand
specification with something more up-to-date.

It also makes some editorial adjustments based on the final
round of RFC Editor updates to RFC 8166.

Lastly it clarifies the use of some of the new error codes
introduced in -04.
