Rich,

I am late to this review; but I have a minor change suggestion.  You use
the word "Reference" in two contexts:


   1.

   2.1.  Reference ID, Kiss-o'-Death,
   4.1.  NTP Reference Identifier Codes


   2.

   Reference (required): the publication defining the ID



As Reference has a specific meaning within NTP, would it be possible to
change the second case to something like "Documented" or "Publication"?
