Looks like  a typo to me,  the two bullets should be 
Continuity Check (proactive)
Connectivity Verification (on-demand)

I see CV more as a commissioning troubleshooting thing, rather than as proactive. It  should of course be possible to use it as  be a proactive function but in its default mode I'd think of it as more of a troubleshooting mechanism.

It is being used synonymously here.

I agree Node E should be a MEP

I am sure one could code/specify ones way around this, I think the main point being made is that is that the presence of MPLS FRR can complicate life if FDI is being used. (Note I am not sure who is proposing to use it but I'm sure will hear on Tuesday)
