Thanks to Carsten's suggestions, i've made this merge request to restore
RFC 4880's four levels deep table of contents:

   https://gitlab.com/openpgp-wg/rfc4880bis/-/merge_requests/32

This is a minor structural tweak, not a substantive change.  hopefully
it helps us avoid a distracting difference from RFC 4880.
