Apparently you gave dish commands equivalent to
	Dish-> moveto "@c=US@st=Massachusetts@o=Boston College"
	Dish-> add
That would try to add `Boston College', which certainly already exists.
You must say
	Dish-> add "cn=Eileen Shepard"
...or whoever you wanted to add.

(If that is not the problem, you must specify which commands you gave
and which dsa you are using, otherwise we can only guess what's wrong.)
