You can't open a stream that exceeds the value your peer sent in MAX_STREAM_ID, STREAM_BLOCKED included.

@mikkelfj, your suggestion to allow use of arbitrary stream identifiers is counter to something we've discussed at length.  The current design allows an endpoint to maintain a map of open streams.  Then the state of other streams is easily determined.  Streams lower than the largest open stream ID are either in the map or are closed, streams with higher numbers are idle.

This is why I think that "fixing" #634 is not a good idea because it means additional state to track closed vs. idle on streams below the largest open stream ID.  And because there is nothing to be gained from opening out of order.  That's even more true with with this change, which removes any vestigial semantics attached to stream identifiers.
