It seems to me that this is the worst case, and that if we think it's fine then every other questions can be considered fine.

My position is that having this worst case is fine.

Many types of greasing (e.g., version negotiation, retry, HelloRetryRequest) introduces one extra roundtrip. So having one extra round-trip when opening one greasing stream sounds not unreasonable.
