@martinthomson is right. The example should be:

* endpoint sends Initial (PN 1)
* after 1 second, endpoint sends two Initials on a PTO (PN 2 and PN 3)
* after 1ms, the endpoint receives an ACK for PN 3

Now we have a loss spanning over 1s, which is >> PTO.
