Just so the change is logged for everyone, I've completely rewritten the `InPersistentCongestion` function based on feedback.  `AreAllPacketsLost` is no longer.

If anyone implements this directly, the (CPU) performance will be terrible, but I believe that it matches the text better.  Anyone interested in how I think this might be implemented without deviating too much from the pseudocode can look at the commit comments, but there are probably much better approaches.
