If you want to add information only to the initial STREAM frame without doubling the number of frames for STREAM, you have some options (each with a tradeoff).

1. As you said, initial STREAM frame (OO=0) can have an additional fixed field.

2. If you need to attach a single bit of information for the initial STREAM frame only occasionally, you can pay 1 byte when that bit of information is present.  (Use the fact that OO=0 and OO=01,Offset=0 are equivalent, so you can overload OO=01,Offset=0 with additional semantics.)  That bit can indicate "UNI" or it can indicate the presence of a fixed field after the Stream ID.

Interesting design.  I was trying to avoid doubling the number of frames consumed by the STREAM frame by consuming just one.  But it's a good insight that offset is only absent on the first STREAM frame, so we could leverage that if we wanted to.  Maybe OO=0 indicates the presence of some fixed-size params, and one of those bits is directionality?
