1.3.5: Add to_be_removed_note 

Kramdown-rfc “---“ sections called note_* turn into notes after the abstract.

RFCXMLv3 introduces removeInRFC=“true” for notes, to indicate that the note is to be removed before publishing as an RFC.  This is now supported (both for v2 and v3) using to_be_removed_note_* (yes, this is, er, even more ugly than the note_* convention in the first place).
