The document that defines the registries (and its successors) defines the rules for putting things in the registry. The DesigExperts are generally just checking that the rules are followed, and perhaps assigning identifiers. This has been my experience as a TLS DE, reviewing and revising the NTP registries, and interacting with the MIME registry.  But there are lots of registries, and maybe some have other rules that gives the DesigExpert more leeway to "just add things."  I'd be curious if you know other cases.  I am more confident that IANA wants rules for it to follow, even if those are FCFS.
