[Speechsc] changes from draft -15 to draft -16

Draft 16 only includes a small amount of completely editorial changes  
needed to address the IANA comments.  Another draft will follow to  
address the remaining review comments.

Here are the changes in this draft.

Added initial registry contents to most of the sections in 13.1 to  
simplify IANA registry creation.
Restructured 13.7 to simplify IANA registry updates.
Merged 13.8 into 13.5 to simplify IANA registry updates.
In the example in section 8.13, corrected Content-Type from "synthesis 
+xml" to "application/ssml+xml".
