This is an update to get out the bulk of edits from Gorry's review and 
some of the other list discussions, but there are a number of topics 
that I think are still converging on the list, so will need a later 
update to catch the results of.  The ones that I'm still tracking as 
open are:

  * Reserved bits
  * Quiet time
  * R1 and R2 discussion
  * Reset generation
  * Variable MTU and MSS
  * IP ID field on retransmits
  * Dead gateway detection
  * Delayed ACKs w/o full segments
  * Comment on OPEN+LISTEN while same local port in in SYN-RECVD/SENT
  * Comment on process IDs
