Done, I think.  Should this registry include the HIT prefix in rfc7343?

I think I have this done.

I will leave this for a later iteration.  I actually put more in about 
ICAO.  I will need to change this somehow to the "organization best 
suited manage the HID space".  Or some such.

BMI1:  This is the first occurrence, in text of "Hierarchical HIT 
(HHIT)" so I believe this is appropriate usage.

In 3.3.1 (and in 5.) your comment says:  "surlignage"  Please translate 
what you are asking done here?  I did look this up in google, but that 
does not help me with the comment.

BMI5 is included for symmetric section construction.  Yes it is 
redundant, but it is there completeness in doc structure.

BMI6 & 7:  Lots of changes made, please review.
