This update addresses Alexey's suggestion that vCard v4.0 be a SHOULD for 
support. In addition to adding that requirement I added a section on 
content conversion - i.e. how clients can request data to be returned in 
one supported format or another. That included adding a new precondition to 
signal when the server is not able to do a conversion, and I also added an 
example of that case.
