While I sympathize with the theory, nobody has reported the bug I found 
which suggests that nobody has ever prepped an I-D.

Whatever we do, we need to be sure the prep tool does what it is supposed 
to.  I've been in touch with Joe and Paul about updating the preptool RFC 
7998 and they say they have some test cases to start from.

Oh, and <toc> definitely needs to go.  Kesara confirms that it's ignored 
when rendering the XML, so it's useless as well as buggy.
