Sorry about two versions, I missed some escaping that needed to be fixed 
immediately for readability.

So a practical diff for this update is between -03 and -05:

Anyway this is the continued work after a longer time of nothing 
happening to the multiplexing guidelines. It is mostly work by Hui Zheng 
and Roni Even. The necessary edits are not complete, but a good start. 
Feedback appreciated, but please note the open issue section with things 
we know we need to consider.
