v-04 https://www.ietf.org/archive/id/draft-ietf-idr-sr-te-policy-attr-04.html has just been uploaded with the following changes.
1) The IANA request  has been updated with the correct name as you pointed out.
2) The references has been updated to be align with the lastest document status.
3) Some minor editorial changes.

 This document requests codepoint allocations for new sub-TLVs of the "Segment List sub-TLV" 
under the "BGP Tunnel Encapsulation".
 
The correct name for the registry is: SR Policy Segment List Sub-TLVs

1. Release an -04 with the correct name for the registry. 
2. Shepherd Review of -03
