Change log:

- Added the Offset parameter as discussed during the interim meeting. It
allows NATs to map a subset of the Internal Port Set that starts from
anywhere in the set (instead of always starting at the Internal Port).

- Clarified text about resource unavailability handling and the nonce
check procedures.
