Issue 79 is mostly resolved by DIGEST-04.  The only thing that is not
covered is the discussion of the RADIUS proxy behavior from the list.  I
think the current text is sufficient unless someone thinks we need to
differentiate RADIUS proxy from RAIDUS server.
