I just did a tidy-up revision after adoption.

Fixed idnits
Fixed references
Fixed spelling
Tidied a little English

No changes of substance, but you are welcome to run a diff.

There are a few empty sections and the authors will try to get to these before
the New Year. If anyone wants to suggest text you may find we say thank-you.
