WG,

This revision is the result of feedback from Flemming during his doc
shepherd write-up process and review.

The changes made are:
- Lots of editorial corrections
- Section 4.2: Added escaping for ICE extensions resulting in new
parameters and values.
- Changed some lower case should, must etc to their upper case versions
where appropriate.
- Section 6.10 has been expanded to clarify when 480 response may result
in freeing of candidates.
- Section 6.12 has text on what to do when a ICE restart fails.
