I'd say it's an implementation issue. If I want to put a call on hold, I will stop sending media no matter what the response to the updated offer is.

Also, as we discussed in SIPCORE when working on the re-INVITE handling spec, clients may not even be able to fallback to the before-the-new-offer-was-sent state (probably not an issue for hold, but in general).

However,  I don't think it's up to us to specify how services work :) We should focus on what happens when no media is sent - for whatever reason.

This is debatable and relates to the discussion about when to apply any change to the session description really the change should probably not be applied until the answer is received otherwise it might be necessary to restore the previous session description depending on the result of the offer/answer exchange.  I would say don't apply the change until the answer is received.
