Nobody is asking for the US disclosure and attestation document. As US 
citizen, lawful resident you have to fill out just the first section and 
sign on the last paper. No need to print it.

I got my negative test, proof of vaccination from my phone (screen shot) 
  and no problem with check-in. They verified the documents at the 
airport and printed the boarding pass
