In my experience, this issue is rather far down the list of things
that non-US Americans are concerned about, other perhaps than the sort
of people looking for an excuse to be offended who you can find
anywhere.

In central and south America, people from the US are often called
"norteamericanos", but then what does one call someone from Canada or
Mexico?  It's not like anyone has trouble understanding what the terms
mean.
