If you want to get philosophical, dual stack was conceived on the assumption that people would be powerfully motivated to migrate because stuff would break due to the shortage of IPv4 addresses. This was before large-scale NAPT44 was considered viable; in that world the path was fairly clear.

I agree that in the world we now live in, -mostly is the best route for many sites, so the Microsoft announcement is welcome.
