I would prefer a model where the chairs add people to the mic line as they
enter it.  I don't want to require the chairs to know every participant's
name, so maybe they can have placeholders such as 'Person 1', 'Person 2',
if they don't know the speaker's name or don't want to enter it.

I do think having the queue management in Meetecho is nice, however, so
both remote and in person participants know where they are in line.
