In the previous release of xml2rfc, the table caption, e.g., "Table 1: Foo"
underneath the table, was always centered on the page in text format,
while the table was left-aligned (the left-alignment was an error, though,
a leftover from the artwork alignment).  That is, the table and caption
was rendered and aligned individually.

As a result of the discussion above, and the table alignment issue, the
latest release does the following:

 1) render the table as a block
 2) render the caption, with wrapping, with the same max width as the table
 3) center the caption under the table, as a new composite block
 4) align the block of table + caption left, center, or right on the page.

The discussion above concerned steps 2 and 3 above, while issues 40 and 9
concerned 4.
