I suspect one problem with standard 3GPP IMS equipment is that it's too expensive and power hungry for smaller locations. :-) 

By all means conformance with laws is required.  In some cases (Sweden, UK? Netherlands?) there are unlicensed bands, which used to be DECT guard bands.  But more typically carriers with spectrum licenses are looking for an economical way to get into rural areas.
